Accessing Your Medical Record
Anyone can request online access to their health care records. Using SytsmOnline you can assess either a basic summary of your record which will include appointment details and a summary of your past and ongoing conditions, or a more detailed summary know as Detailed Coded Records (DCRA) which includes more clinical information such as blood test results and investigations.
(Please note, test results will only be available to view after they have been reviewed by a clinician)
To gain access to your summary record:
If you are already registered for online access you can request access to your summary record via your online account or in person at any of our sites. This will almost always be granted within 1 working week.
To gain access to your detailed coded record:
If you are already registered for online access you can request access to your detailed coded record via your online account or in person at any of our sites. Access cannot always be guaranteed and processing can take up to 28 working days.
If you are not registered for online access and wish to do so, please complete the form below and bring 2 forms of recognised (preferably photo) ID to one of our sites to apply for access.
You may also use the form below to apply for “Proxy Access” to another patient’s record, such as a family member or someone you care for.

Some details within your health records may be exempt from disclosure, however this will in the interests of your well-being or to protect the identity of a third party.
