Embracing Life

This equipment was formally presented to the Partners and Nurses of Chesterfield Medical Partnership on Wednesday December 17 2014 at the Ashgate Manor Site.

Chesterfield Medical Partnership is delighted to receive support from Embracing Life in the form of state of the art defibrillators for cardiac emergencies and spirometry equipment which integrates with the clinical records system to assist with Chronic Obstructive Pulmonary Disease diagnosis and monitoring.

Patient Access to their GP Record

In line with our statement of intent posted on this website and at each of our surgeries on 26th September 2014:

We now offer the facility for patients to view online, export or print any summary information from their records i.e.

  • Medications
  • Allergies
  • Adverse reactions
  • Any additional information agreed between the GP and patient

If you are already registered for SystmOne online for repeat prescription ordering or appointment booking, please ask at reception to have this facility added.

If you are not registered, and would like this access, please bring 2 forms of recognised ID to reception to receive your logon and password.

A new way to get your prescriptions goes LIVE at Chesterfield Medical Partnership on Tuesday September 9 2014.

The Electronic Prescription Service is an NHS service. It gives you the chance to change how we send your prescription to the place you choose to collect your medicines from. To nominate, please take this FORM to the Pharmacy you wish your Electronic Prescriptions to go to.

Are you aged 75 or over?

You may be aware that from April all practices are required to provide all their patients aged 75 and over with a named GP, who will have overall responsibility for the care and support that our surgery provides to them. This does not prevent you from seeing any GP in the practice as you currently do.

Please ask at Reception for the name of yours.
