Accessing Your Medical Record

Anyone aged 16 and over can request online access to their summary records OR their detailed coded records (DCRA). The latter will include test results after the clinician has seen and reported on them.

To gain access to your summary record:

If you are already registered for online access you can request this via your online login or at any of our 3 sites. This will almost always be granted within 1 working week.

To gain access to your Detailed coded record:

If you are already registered for online access you can request this via your online log in or at any of our 3 sites. Access cannot always be guaranteed and can take up to 28 working days.

If you are not registered for online access and wish to do so, please bring 2 forms of recognised (preferably photo) ID to one of our three sites along with the consent form (please click on link below) to apply for this access.

To gain access for your Full Medical Records:

If you require a full copy of your records you are required to make a “subject access request” in writing (please click below for our leaflet regarding this). Please note there is a charge for this service of up to £50.
